Membership is billed as an annual fee. All benefits, terms, exclusions, and discounts are governed by the membership agreement. Please note: this plan is not dental insurance.

Preventative Plan:
- Two preventative hygiene visits
- Two wellness exams
- All necessary x-rays
- Two fluoride treatments
- One emergency exam
- 20% off dental treatment (smile design procedures excluded)
Periodontal Plan:
- Four periodontal maintenance visits
- Two wellness exams
- All necessary x-rays
- Four fluoride treatments
- Four irrigation and laser treatments
- 20% off dental treatment (smile design procedures excluded)
Membership Plan Fees
Periodontal Plan:
- Family membership: Enroll two or more family members and each additional member saves 10% on their annual membership fee — a value of $1,500 on the Periodontal Support Membership.
- Specialist savings: Members save 20% with our select specialist partners, including AZ Perio, Canyon Ridge Endo, and Galati Orthodontics.
Preventative Plan:
- Family membership: Enroll two or more family members and each additional member saves 10% on their annual membership fee — a value of $800 on the Preventative Plan Membership.
- Specialist savings: Members save 20% with our select specialist partners, including AZ Perio, Canyon Ridge Endo, and Galati Orthodontics.
